JOB TITLE: Licensed Vocational Nurse (LVN)
JOB SUMMARY:
Responsible for delivery and supervision of resident care through the nursing
process of assessment, planning, implementation and evaluation. Supervises
direct resident care personnel. Prepares and maintains records and reports as
required.
JOB QUALIFICATIONS:
A. Education: Graduate of an accredited School of Vocational Nursing.
B. Personal Job-Related Skills: Advanced knowledge of nursing theories and practices.
Leadership and management abilities and effective oral and written communication skills.
C. Licensure, Registry or Certification: Registered Vocational Nurse currently licensed by the State of Texas. Must continue to complete annual education requirements.
D. Experience:
1 . Prior Work Experience: Preferred to have one year of nursing experience in long term
care or a related field as desirable.
2. Technical Training: Current CPR certification.
3. Physical and Mental Requirements: Duties of the position require intermittent
walking, and standing, pushing, pulling, reaching, and bending when involved in
patient care. Lifting up to a weight of 50 pounds and occasional heavy lifting in
supporting or moving patients that may suddenly become weak or helpless.
ESSENTIAL DUTIES AND RESPONSIBILITES:
A. Responsible for the delivery and supervision of resident care.
1. During the sift, assumes responsibility for the total nursing care of residents.
2. Recognizes significant changes in the condition of residents and takes necessary action.
3. Ensures that the individual nursing care plans are followed to meet the resident’s needs.
4. Ensures that drugs and medications are procured, inventoried, prepared, administered and charted according to established policies and procedures.
5. Administers and supervises prescribed treatments.
6. Makes rounds with physicians and records visits in clinical records.
B. Provides supervision and assistance and performs support duties as needed.
1. Supervises direct patient care personnel which includes corrective and disciplinary actions, support and encouragement, and enforcing responsible behavior.
2. Monitors the serving of prescribed diets and fluid intake and reports persistent unresolved problems to the physician and the Director of Nursing.
3. Ensures the preparation of accident/incident reports and follows through with the required procedures.
4. Ensures that responsible party, family members and attending physician are kept timely and properly informed of residents changing condition.
5. Ensures that all resident care is documented according to established policy and procedures.
6. Ensures that a copy of the medical record or an abstract of them is forwarded with the transfer of a resident.
7. Assists in maintaining a clean, safe environment for residents, detecting and correcting situations that have a high probability of causing accidents or injuries to residents.
8. Handles problems well independently and recognizes when a situation requires assistance from proper personnel when necessary.
9. Participates in the Interdisciplinary Care Plan meetings as required.
10. Receives verbal or telephone orders from physicians.
11. Ensures continuing promotion of residents’ physical and emotional health by assisting and teaching them .
OTHER SIGNIFICANT REQUIREMENTS:
A. Maintains confidentiality while working with confidential matters on a daily basis.
B. Provides service direct to patients while maintaining a positive patient relations atmosphere.
C. Interacts appropriately with patients, family members, Medical Staff members and other department managers on a continuous basis.
D. Follows aseptic procedures and procedures for using precautions when involved in direct patient contact or handling potentially infectious materials.
E. Follows safety guidelines and policies for the Center.
